In answer to question  #1:
This looks very much like the other issue you had with caching. I'm making the assumption that you have cleared the cache and that this is another issue you are having after having cleared the cache.

If you can attach the rhq.js file we can see if something is corrupt. That file is at: 
http://<server>:7080/coregui/js/rhq.js

Also, can you provide the Browser version and platform as well?
