Now I realized that I should not forgot to announce, that in RHEL 6.5 was released rubygems errata [1], which adds rubygems-devel subpackage with all rubygems macros you are used to already for several releases of Fedora. Namely, you should be able to use
%gem_dir
%gem_instdir %gem_libdir %gem_cache %gem_spec %gem_docdir
%gem_install

If somebody cares to backport these macros into EPEL 5, feel free to assign yourself to [2].
BTW I opened FPC ticket to update the Ruby packaging guidelines.

Speaking of EPEL6, you still need some fancy macros to cover the ruby(abi) vs ruby(release) difference, but that is all the difference.
